ITEMS PATIENTS NEED TO BRING DURING THEIR VISIT

Please download the Initial Evaluation Form, fill it up and bring it on your visit. Accomplishing this form will greatly help your physician in understanding your physical condition.

  • X-rays, CT scans and MRI's - please bring the actual films if possible

  • All laboratory results and old records especially operative report

  • For female patients with Shoulder problems

         - tank top or tube top

  • For patients with Knee problems

         - loose fitting pants or shorts
